You wear it in place of a suit. I found they ran small.

Q: Would I be ok with 12-18 months if my 15 month old is only 18 pounds? Or should I go down a size?

I wouldn't go down a size. My 18 month is only 20 pounds and I got the 18-24 and it fits fine
