Q: Will this work with flashlight and red dot attachments?

Probably not for a .45, but I don't know. I don't have those attachments.

Lifella elastic holster can fit up to any handguns that you own whether it is full size or small size and it will keep it tightly secured with the strong detachable button strap but also allow you to quickly draw your pistol.

Q: How does this work for a fat person

Not sure I'm fat, 6 ft 2 in and 240 lbs, but it works well for me. You can place this just about however you'd like and my one complaint is that it can be hot to wear. When temperatures are above 75 degrees, I'll use one of my other holsters.

Q: Will the bullets fall out when running?

Bullets fall out of what, if you are refering to the magazine it has a strap to hold it in.
