I’ve had it for probably 6 months. **Q: Does this have a surge protector?** A: No it’s doesn’t have a surge protector **Q: Is the back 2 prongs or 3 prongs?** A: 3. If there is three on the front there is three on the back. The round one is for the ground, the longer slot is for the neutral, and the shorter slot is for the hot leg. 120 volts between long and short openings. Unit plugs right into a three prong outlet over the wall plate and the center screw secures it in place. I bought four of these and they're excellent. I was an electrician for 37 years and I'll vouch for them. However you do need a three prong outlet. The ground prong (round) is very important. **Q: What is the height of the adapter?
